Program pathways

The following flowchart sets out the process by which participants can be referred into the Life! Program.

For patients 50 years and above and all adult Aboriginal and Torres Strait Island people

Patients with a history of CVD/GDM or who have had a WorkHealth check

People aged 18+ may be referred to a Life! course through the WorkHealth initiative if they are at high risk of diabetes. People with a history of a high risk condition such as CVD/GDM are also eligible at any age 18 years and over.
